A Case of Wegener¡¯s Granulomatosis Presenting as Otitis Media

Abstract
Wegener¡¯s granulomatosis is an uncommon idiopathic, multi-system disease characterized by granuloma formation and vasculitis affecting small to medium sized vessels. It usually presents with involvement of upper, lower respiratory tracts and the renal system, but also may include otological manifestations. A 54-year-old woman was suffering from right otorrhea and ear fullness and treated with antibiotics. Despite of the treatment, the otologic symptoms were aggravated and right facial palsy was developed. She also represented rhinologic symptoms such as nasal obstruction and rhinorrhea after 2 months. Diagnosis as Wegener¡¯s granulomatosis was supported by biopsy from nasal cavity and serologic marker such as C-ANCA. She underwent medical therapy with prednisone and cyclophosphamide, and then both otologic and rhinologic symptoms were improved. We report this case with a review of the related literatures.
